== Trigger mechanism == Insulin is synthesized and secreted in the beta cells of the islets of Langerhans. Once insulin is synthesized, the beta cells are ready to release it in two different phases. As for the first phase, insulin release is triggered rapidly when the blood glucose level is increased. The second phase is a slow release of newly formed vesicles that are triggered regardless of the blood sugar level. Glucose enters the beta cells and goes through glycolysis to form ATP that eventually causes depolarization of the beta cell membrane (as explained in Insulin secretion section of this article). The depolarization process causes voltage-controlled calcium channels (Ca2+) opening, allowing the calcium to flow into the cells. An increased calcium level activates phospholipase C, which cleaves the membrane phospholipid phosphatidylinositol 4,5-bisphosphate into Inositol 1,4,5-trisphosphate (IP3) and diacylglycerol (DAG). IP3 binds to receptor proteins in the membrane of the endoplasmic reticulum (ER). This releases (Ca2+) from the ER via IP3 gated channels, and raises the cell concentration of calcium even more. The influx of Ca2+ ions causes the secretion of insulin stored in vesicles through the cell membrane. The process of insulin secretion is an example of a trigger mechanism in a signal transduction pathway because insulin is secreted after glucose enters the beta cell and that triggers several other processes in a chain reaction.

In addition, progesterone-like steroids are found in the plant Dioscorea mexicana, part of the yam family native to Mexico. Dioscorea mexicana contains a steroid called diosgenin which is taken from the plant and converted into progesterone. Diosgenin and progesterone are also found in other Dioscorea species, as well as in other plants that are not closely related, such as fenugreek. Another plant that contains substances readily convertible to progesterone is Dioscorea pseudojaponica, native to Taiwan. Research has shown that the Taiwanese yam contains saponins—steroids that can be converted to diosgenin and thence to progesterone. Many other Dioscorea species of the yam family contain steroidal substances from which progesterone can be produced. Among the more notable of these are Dioscorea villosa and Dioscorea polygonoides. One study showed that the Dioscorea villosa contains 3.5% diosgenin. Dioscorea polygonoides has been found to contain 2.64% diosgenin, as shown by gas chromatography-mass spectrometry. But with the mass exodus at the Pan-European Picnic the subsequent hesitant behavior of the ruling Socialist Unity Party of East Germany and the non-interference of the Soviet Union broke the dams. Now tens of thousands of media-informed East Germans made their way to Hungary, which was no longer willing to keep its borders completely closed or to oblige its border troops to use armed force. On the one hand, this caused disagreement among the Eastern European states and, on the other hand, it was clear to the Eastern European population that the governments no longer had absolute power.

=== Microanatomy === The thymus consists of two lobes, merged in the middle, surrounded by a capsule that extends with blood vessels into the interior. The lobes consist of an outer cortex rich with cells and an inner less dense medulla. The lobes are divided into smaller lobules 0.5-2 mm in diameter, between which extrude radiating insertions from the capsule along septa. The cortex is mainly made up of thymocytes and epithelial cells. The thymocytes, immature T cells, are supported by a network of the finely-branched epithelial reticular cells, which is continuous with a similar network in the medulla. This network forms an adventitia to the blood vessels, which enter the cortex via septa near the junction with the medulla. Other cells are also present in the thymus, including macrophages, dendritic cells, and a small amount of B cells, neutrophils and eosinophils. In the medulla, the network of epithelial cells is coarser than in the cortex, and the lymphoid cells are relatively fewer in number. Concentric, nest-like bodies called Hassall's corpuscles (also called thymic corpuscles) are formed by aggregations of the medullary epithelial cells. These are concentric, layered whorls of epithelial cells that increase in number throughout life. They are the remains of the epithelial tubes, which grow out from the third pharyngeal pouch of the embryo to form the thymus.

== Causes == Researchers have mapped Urbach–Wiethe disease to chromosome 1 at 1q21 and specifically identified the extracellular matrix protein 1 (ECM1) gene as the gene containing mutations that can lead to the development of the condition. At this point, 41 different mutations within ECM1 have been reported to lead to Urbach–Wiethe disease. These were all homozygous loss-of-function mutations (i.e. nonsense, frameshift or internal deletions). It is an autosomal recessive condition, requiring two mutated copies of the ECM1 gene to cause the disease. ECM1 codes for a glycoprotein of previously unknown origin. The discovery that the loss of ECM1 expression leads to the symptoms associated with Urbach–Wiethe disease suggests that ECM1 may contribute to skin adhesion, epidermal differentiation, and wound healing and scarring. It is also thought to play a role in endochondral bone formation, tumor biology, endothelial cell proliferation and blood vessel formation. The dermatological symptoms are caused by a buildup of a hyaline material in the dermis and the thickening of the basement membranes in the skin. The nature of this material is unknown, but researchers have suggested that it may be a glycoprotein, a glycolipid, an acid mucopolysaccharide, altered collagen or elastic tissue.
